Design my 10-page checklist ebook

I have an e-book which I am planning to give away for free to all those who subscribe to my blog.

The e-book is basically a checklist type and is written to be the shortest possible style.

I want it to be designed and decorated professionally, including a unique cover for the e-book.

To be more specific, the design should be made using brown color as the main color, and should be attractive.

There is one important requirement too: It should look like a checklist!

To all those who think that the price is low, let me say it again. The ebook is only of 10 pages, and probably wouldn't take much time and efforts to be designed.

Requirements

I will keep it simple and straightforward:

• You should design the cover of the checklist separately.
  
• You should design the checklist, taking brown as the main color. The reason is that, my logo and blog template are also brown.
  
• You should add various elements and clip-arts to make it look professional
  
• You should use creative fonts each for heading and text.
  
• I don't mind what you use to decorate the e-book. I will give you the ebook in word document form and you should return the checklist in word document form only. If not possible, I can settle with PDF format.
  
• It should look like a checklist!
  

I have attached a snapshot of a similar checklist. I want something like that, but having your own set of fonts and elements.
