Need a one Page Simple PHP Code to upload image

I want a one-page PHP code where [no MySQL needed]. Just the page where there will be a built-in username and password and when I will log in and can upload images. The image will be stored on my web hosting under my domain.
Guess my website is
www.mydomain.com

I want to login using
www.mydomain.com/admin.php

The images I will pick will be stored in
mydomain.com/images/uploadedimage.jpg
