I will create PDF form from contents you provide or files that may be any of word, excel, scanned file or whatever you have. I will integrating form fields as per your requirement, like: drop down menus, text box, digital signature, check/tick boxes, date etc.
This gig will cover only one page. If you have multiple pages, you will require to order multiple quantity according your document length. Or contact me before ordering the item.
If you have any calculation in the form you will require to order extra item of "add a calculation table". Table may include: Income and Expenditure sheet, order table, P&L account or even any fields they need to be calculated. You can discuss with me before ordering this gig.
